Owner Liability Waiver & Permission

Health Certification:

By participating in services at Nature In Play, Inc, I hereby certify that my dog(s) is/are in good health and has/have not been ill with any condition that will/should limit their ability to participate in the services.

Safety Precautions:

I understand that Scott Elliott and Nature In Play, Inc will make sure safety comes first by taking the proper precautions, like using muzzles, but I acknowledge things can happen when working with dogs that have aggressive or other behavioral problems.

Liability Responsibility:

I further understand that I am solely responsible for and agree to indemnify Scott Elliott and Nature In Play, Inc from all liability incurred as a result of any injury and/or harm to a person or another dog caused by my dog(s) while in the custody of Scott Elliott, his employees, and volunteers at Nature In Play.

Release and Discharge:

In consideration of the services provided by Nature In Play, Inc its owners, officers, employees, agents, volunteers, participants, and all other persons acting in any capacity on its behalf, I hereby release, and discharge Scott Elliott and Nature In Play, Inc as follows:

Acknowledgment of Risks:

I acknowledge that working with, and/or being in the presence of, aggressive dogs or dogs with other behavioral issues, involves known risks which could result in physical or emotional injury, paralysis, death, or other injury to myself, to third parties, or to property. The behavior of animals cannot be predicted with absolute certainty under all circumstances; therefore, I acknowledge that there is a possibility of injury from dog bites, scratches, or other contacts with dogs. There is also a possibility of injury resulting from handling of dogs or dogs that may break loose from their restraints or enclosures.

Assumption of Risks:

I further acknowledge that the description of possible risks is not complete and that other unknown or unanticipated risks could result in injury to person or property, or death. I expressly agree and promise to accept and assume all of the risks involved in this activity. My participation is purely voluntary, and I elect to participate in this activity knowing of the risks.

Release of Claims:

I hereby voluntarily release, forever discharge, and agree to indemnify and hold harmless Scott Elliott and Nature In Play, Inc from any and all claims, demands, or causes of action which are in any way connected with my participation in this activity or my use of Nature In Play facilities, including any such claims which allege negligent acts or omissions of Nature In Play.

Insurance Certification:

I certify that I have sufficient insurance to cover any injury or damage that I may suffer while participating in this activity, or I agree to bear the costs of such injury or damage myself. I further certify that I have no medical or physical condition which could interfere with my safety in this activity, or I agree to bear the costs of all risks that may be created, directly or indirectly, by any such conditions.

Permission for Media Use:

I grant permission to Nature In Play, Inc to make movies, videos, and photographs which may include images of myself and to use them for promotional or commercial purposes without recourse or compensation to me.

Financial Responsibility for Veterinary Treatment:

I further understand and agree that in the event that my dog becomes ill or is injured while at Nature In Play, Inc facility, I will be responsible for any and all expenses for veterinary treatment and related costs. Staff at Nature In Play will attempt to notify me prior to seeking veterinary treatment for my dog(s) where circumstances permit. However, in emergency situations, treatment may be sought without prior notification.

Acknowledgment on Rehabilitation:

I further understand and agree that rehabilitation depends at least in part on a dog’s ability to change; Results are not guaranteed.

Certification of Understanding:

I certify that I have read and understand the foregoing terms and conditions and am at least 18 years of age. I agree to abide by the rules and accept all terms, conditions, and statements of this agreement.
